STUDENT ACCOUNTS FORMS

pdf Student Financial Agreement and Disclosure Form
To be completed by the student (and signed by a parent if student is under 18). This form is required from all students in order to maintain registration and allow payment at a time other than when processing the registration form.

Financial Information Release Form
To be completed by the student. This authorizes Warner Pacific College to share confidential student account information with persons permitted by the student. This form does not give permission for academic information (i.e. grades).

Cash Management Authorization Form
This form should be completed by the student. It identifies for Warner Pacific College, the way students wish to manage the funds on their student account.

pdf Payment Plan Authorization Form
This form is used to sign-up for an automatic payment plan to cover any balance on the student account. For information about filling out the form, or any questions about figures, please contact the Office of Student Financial Services at 503-517-1207.

FINANCIAL AID FORMS

2013-14 Verification Forms
Some students may be selected to complete the Federal FAFSA Verification process. These forms are provided for easy access to those students. Please only complete this process if notified of selection. Please refer to the Student Aid Report to identify which forms to complete.

Dependent Verification Worksheet
For students who must include parent information on the FAFSA.

Independent Verification Worksheet
For students whose parents are not required to provide information on the FAFSA.

Child Support Paid Worksheet
For students selected for verification who paid child support in 2012.

Identity and Educational Purpose Worksheet
This form should be completed in person at the Warner Pacific College Financial Aid Office or in the presence of a Notary if you are unable to appear iin person at Warner Pacific College.

13-14 FAFSA Waiver
All students required to complete and submit a 2013-2014 Free Application for Federal Student Aid (FAFSA) in order to be able to register for classes. Some students may be eligible to waive this requirement. Only use this form if any type of Federal Financial Aid (including the loan programs) will not be used. Please contact the Office of Student Financial Services or an Admissions Counselor before completing this form.

13-14 Special Conditions Appeal
This form should be completed in consultation with the Office of Student Financial Services. Some students may be eligible for an adjustment to their FAFSA if the student or the family’s financial situation has changed since 2011. Please contact the Office of Student Financial Services at 503.517.1091 for guidance on whether this process would be beneficial.
